The frequency of home nursing visits is determined based on each individual's specific care needs, as assessed by our professional healthcare team. Care plans are customized to ensure the appropriate level of support, with visits scheduled daily, weekly, or as needed.
A registered nurse or qualified healthcare professional will assess the individual's health condition, medical history, and care requirements. Factors such as the severity of the condition, the complexity of care, and any existing support services are considered when determining visit frequency.
For those requiring complex medical procedures or intensive care, multiple visits per day may be necessary. Conversely, routine care or monitoring may only require visits once or twice a week.
Our care plans remain flexible and adaptable, adjusting over time to meet changing health conditions and evolving care needs. The frequency of nursing visits can be modified as required to ensure optimal support.
Home care services are funded in the greater Darwin Palmerston NT area through a combination of government subsidies and individual contributions, with the Australian Government contributing a portion of the cost through Home Care Packages, and individuals contributing through fees.

The Nurse Consultant will develop and oversee educational programs, provide hands-on mentorship, and collaborate with healthcare teams to drive quality improvements in home care practices.
Key Responsibilities:
Educational Program Development:
Design, develop, and implement educational programs for training home care providers who assist elderly individuals and those with disabilities.
Develop learning building blocks on topics like clinical deterioration, personal care specific tasks, medication management, mobility support, palliative care, and mental health for clients with disabilities.
Ensure that training materials and programs are evidence-based, tailored to individual care needs, and comply with healthcare regulations and standards.
Mentorship and Supervision:
Provide mentorship and hands-on supervision to support care providers, guiding them through best practices in patient care, including hygiene, mobility, and medication management.
Work closely with caregivers during home visits or clinical placements to ensure they apply theoretical knowledge to practical, real-world scenarios.
Conduct regular assessments of caregivers’ performance and provide constructive feedback to improve their clinical practices.
Age and Disability Home Care Services Support:

Help caregivers implement and adhere to infection control and safety protocols within the home environment.
Companionship and Emotional Well-Being:
Train caregivers in fostering social interaction and emotional support, addressing the importance of companionship and mental health for clients who may experience isolation or depression.
Offer guidance on creating meaningful, engaging activities for clients, promoting independence, and improving overall well-being.

Qualifications and education:
Registered Nurse (RN) with a valid nursing license AHPRA
Grad Dip in clinical education, gerontology or disability
Graduate-level qualifications Clinical Education.
Additional certifications in palliative care, dementia care, or related fields are highly advantageous.
Experience: Hospital in the home, clinical lead for age care, community health rural nursing lecturer, disability support mentor and champion
28 years plus as Registered nurse, clinical nursing and teaching experience, with a strong background in home care, geriatric care, and disability support.
Experience in designing and delivering educational programs for healthcare professionals, especially those in home care or community health settings.
Experience in mentoring and supervising caregivers or home care staff.
Skills and Competencies:
